Online forms and 'PDF' forms help meet service needs
The effectiveness of Lakewood customer service is enhanced by a collection of online forms targeted at the city's most requested services. General service requests and questions can be made by submitting the main general service request form.

More unique service needs like graffiti removal, or reporting a broken street light, can be dealt with via specific online forms. They make it easier to fill in the specifics needed to more quickly resolve those requests.

Online forms are listed on the main "Forms" page under the "Services/Forms" button at the top of most site pages. Also, look for the Acrobat PDF button.

'Real' signatures sometimes are required

Lakewood Online's electronic forms, that are filled out online, are an effective way of contacting our customer service staff. But, there are certain instances where the City of Lakewood may need a 'real ink signature' on an official document.

For example, city crews need property owner permission to paint out graffiti on private property. While a graffiti removal request can be made online, a graffiti access release form still needs to be filled out, signed, and then mailed, or walked into city hall's customer service staff.

An original signature of the property owner is required to give city crews access to a specific private property to paint out graffiti. A release is good for a year.
